FORM: Request to Withhold Directory Information: “Opt Out” Request Form and Photo/Video Release

Dear Families,

Throughout the course of the school year, the District may release some basic student information - "Directory Information" - without the consent of an Eligible Person (Parent/Guardian/student 18 years of age or older). Directory Information (including photographs) may be published in a variety of ways, such as in school yearbooks, on the District website, or released to news media/social media. 

If you do not want your student’s Directory Information released or published, please complete this Directory Information "Opt-Out" Request Form within five (5) school days of the start of the academic year. If an "Opt-Out" Request is not received by the District on or before that date, consent to release of such information will be assumed.

Note: The District cannot control the release of certain Directory Information, such as digital images or names, when students participate in school events open to the general public, including but not limited to, athletic competitions, concerts and plays.

Additionally, please be advised that during the school year your student may be photographed and/or videotaped at various school sponsored events. With your consent, the photograph or video may be reproduced and released for use in the media, School produced brochures, newspaper, videos, the internet and School or District websites, and social media platforms.

By granting general permission in advance, you are simply allowing for your student to be photographed or videotaped by the school staff, the principal, the local newspaper, or local cable station throughout the year. If for any reason you do not wish for your student to be photographed or videotaped, please indicate your preference in the attached form.
